Dental and Vision
  • New Dental and Vision Plan Premium Tier Structure
The dental and vision plan premium tier structures are changing to align more closely with that of the medical plan. The table below summarizes the change in levels, which could result in your having a different coverage option in 2018 than in 2017, depending on those you're covering.
THOSE YOU PLAN TO COVER IN 2018
YOUR 2018 DENTAL PLAN COVERAGE LEVEL
You only
Employee
You plus one child
You plus more than one child
Employee + Child(ren)

Employee + Spouse/Domestic Partner
You plus your spouse/domestic partner and child(ren)

Your dental and vision plan coverage level will be automatically defaulted into the tier that matches the coverage you have today and you'll pay the 2018 contributions for that level. If you would like to make a change to your tier or coverage, please be sure to review your benefits and make changes during the 2018 Open Enrollment period.
  • MetLife Preferred Dental Program Coverage Changes
    • Today, out-of-network usual, reasonable and customary ("URC") charges are charged at the 90th percentile which means that 90 percent of dentists in your geographic area charge that fee or less. To encourage use of in-network providers, beginning in 2018, out of-network URC charges will be based on fees charged at the 80th percentile.
    • While non-surgical periodontal services will continue to be classified and paid as Basic Services, surgical periodontal services will be reclassified and paid as Major Services under the Plan.
    • The missing tooth exclusion will be eliminated, enabling enrolled individuals to receive coverage as defined under the Plan for eligible services related to the loss of teeth prior to coverage under the Plan.
